Fee Schedule Report

Now accepting BCBS plans, other Insurance billing will be provided upon request but payment must be made day of service. Under certain circumstances or hardships, special pricing and payment arrangements can be made.

Office Information Report

I am available for home visits as well as treatments at hospitals and rehabilitation facilities. However I do charge for travel time and setup. When arriving at the office for your first appointment please take a seat in the waiting area. I am usually on time, and will let you know if I am running late. Currently payment is only accepted in check, credit card, cash or with the banking app Zelle.
